Medical & Mental Health Disclaimer
Medical & IV Hydration Disclaimer
1. Elective Wellness Only; Not Emergency Care The intravenous (IV) hydration, vitamin infusions, and nutritional injection services offered by Reboot IV Therapy are designed strictly for elective wellness, hydration support, and nutritional supplementation. We do not provide emergency medical services. If you are experiencing acute dehydration, chest pain, severe shortness of breath, or any other medical crisis, do not use this website or seek our services. Call 911 or report to the nearest Pennsylvania emergency room immediately.
2. No Provider-Patient Relationship via Website Browsing this website, reading our content, or filling out a contact form does not establish a formal medical provider-patient relationship. A formal clinical relationship is only established after you have undergone a formal medical screening, and a licensed Pennsylvania healthcare professional has approved you for treatment.
3. Medical Screening & Right to Refuse Service Because IV therapy involves the administration of prescription-grade fluids and nutrients, all clients must clear a clinical health screening prior to receiving an infusion. Reboot IV Therapy reserves the absolute right to refuse service to any individual if our medical staff determines a treatment is contraindicated based on your health history (such as pre-existing kidney disease, congestive heart failure, or fluid retention issues).
4. FDA Limitation & Results Disclaimer You acknowledge that the vitamins, minerals, and compounds used in our IV formulations are not evaluated or approved by the U.S. Food and Drug Administration (FDA) to diagnose, treat, cure, or prevent any specific disease or medical condition. Individual results vary, and no clinical outcomes are guaranteed.
5. Client Duty to Disclose Accurately You agree that you are solely responsible for providing complete, truthful, and accurate information regarding your medical history, current prescription medications, allergies, and physical health status during your medical intake. Reboot IV Therapy is not liable for adverse reactions resulting from a client’s failure to disclose relevant health data.
Terms and Conditions
1. Acceptance of Terms By accessing and utilizing the website of Reboot IV Therapy, you agree to comply with and be bound by these Terms and Conditions. If you do not agree, please discontinue use of this site immediately.
2. Age Requirements Our services and website are intended solely for individuals eighteen (18) years of age or older. By requesting an appointment, you certify that you meet this age requirement.

Cancellation, Late, and No-Show Policy Because our specialized IV formulations and vitamin mixtures are custom-prepared for your specific appointment time, late cancellations (less than 24 hours’ notice) or no-shows cause a direct loss of medical product and clinical staff time. Reboot IV Therapy reserves the right to charge a cancellation fee up to the full price of the scheduled service for missed appointments.
4. Limitation of Liability To the maximum extent permitted by the laws of the Commonwealth of Pennsylvania, Reboot IV Therapy, its Medical Director, officers, employees, and independent contractors shall not be liable for any direct, indirect, incidental, or consequential damages resulting from the use of, or inability to use, this website or the information contained herein.
5. Governing Law These Terms and Conditions, and any disputes arising from your use of this website or our services, shall be governed by, construed, and enforced in accordance with the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles.
